Minor Volunteer Release and Waiver of Liability

This release and waiver of Liability (the “Release”) executed on this ______ day of ______________, 20__, by _______________________, the legal guardian (the “Guardian”) of ____________________, a minor child (the “Minor”) releases the Junior League of Columbia, a nonprofit corporation organized and existing under the laws of the State of South Carolina, and its directors, officers, employees, and agents (collectively, the “JLC”).

The Minor ________________________, desires to participate in the JLC’s Junior Volunteers program.  As such, the Minor desires to volunteer for the JLC and engage in the activities related to being a volunteer for the JLC. I, the Guardian ___________________________, hereby give my consent to the Minor’s participation in the JLC’s Junior Volunteers program.  Additionally, I freely and voluntarily, without duress, execute this Release on behalf of myself and the Minor, under the following terms:

1. Waiver and Release. I, individually and on behalf of the Minor, release and forever discharge and hold harmless the JLC and its successors and assigns from any and all liability, claims, and demands of whatever kind or nature, either in law or in equity, which arise or may hereafter arise from the Minor’s volunteer work for the JLC. I, individually and on behalf of the Minor, understand and acknowledge that this Release discharges the JLC from any liability or claim that I or the Minor may have against the JLC with respect to bodily injury, personal injury, illness, death, or property damage that may result from participation in the Junior Volunteers program and/or other JLC events.
2. Insurance. I affirm that the Minor is covered by primary medical insurance and understand that I, the Guardian, am responsible for the Minor’s medical bills if an injury occurs. Further, I, individually and on behalf of the Minor, understand that the JLC does not assume any responsibility for or obligation to provide financial assistance or other assistance, including but not limited to medical, health, or disability insurance of any nature in the event of injury, illness, death or property damage.
3. Medical Treatment. I, individually and on behalf of the Minor, hereby release and forever discharge the JLC from any claim whatsoever which arises or may hereafter arise on account of any first-aid treatment or other medical services rendered in connection with an emergency during the Minor’s time with the JLC. Further, I give my consent for medical treatment of the Minor by the closest hospital, doctor, or medical facility should injury occur.
4. Assumption of Risk. As Guardian, I recognize and understand that the Minor’s volunteer service with the JLC may, in some situations, involve inherently dangerous activities. As the Guardian for the said Minor I hereby expressly assume the risk of injury or harm from these activities and release the JLC from all liability for injury, illness, death, or property damage resulting from the services the Minor provides as a volunteer or occurring while the Minor is participating in the JLC’s Junior Volunteers program.
5. Photographic Release. As the Guardian of Minor, I grant and convey unto the JLC all right, titles, and interests in all photographic images and video or audio recordings made by the JLC during the minor’s work with the Junior Volunteers program.
6. Transportation. I, individually and on behalf of the Minor, hereby release and forever discharge the JLC from any claim whatsoever which arises or may hereafter arise while the Minor is en route to or returning from a Junior Volunteers activity.
7. Scope of Release. I, individually and on behalf of the Minor, expressly agree that this Release is intended to be as broad and inclusive as permitted by the laws of the State of South Carolina in the United States of America and that this Release shall be governed by and interpreted in accordance with the laws of the State of South Carolina. I agree that in the event that any clause or provision of this Release shall be held to be invalid by any court of competent jurisdiction, the invalidity of such clause or provision shall not otherwise affect the remaining provisions of this Release which shall continue to be enforceable.
